Missouri School Sports Physical Requirements

Missouri athletes need a physical signed by a licensed provider (ie. MD, DO, ARNP, PA, or DC) to participate in:

  • Middle school sports
  • High school sports (MSHSAA)
  • Marching band
  • Club team sports
  • Cheerleading, dance, and gymnastics

What is a sports physical?

A sports physical, also known as a pre-participation physical exam, helps to ensure your child or teen is healthy and physically prepared for school, club, or camp sports. During the visit, we will go over:

To make your appointment as smooth as possible, please remember to bring the required physical form, pre-filled out by you, with information on:

  • Previous injury or surgery notes

Typically they take 20-30 minutes. If you do not have the sports physical form filled out, please arrive 10 minutes earlier than the appointment time.
